About the Speaker: Manon Carter, LCSW Social Worker Supervisor
Manon Carter is a medical social worker who has been employed with Stanford Health Care Tri-Valley for over 25 years. She is a Licensed Social Worker with certifications in Palliative Care and Advanced Certification in Social Work and Case Management. She believes that patients should be proactive in making their health care decisions known via a health care directive, living will, and/or Physicians Order for Life Sustaining Treatment form (POLST).
